The type of personal information we collect
We currently collect and process the following personal information:
- Name
- Email address
- Questions/comments about ACED Cohort Study (may contain personal information)
- Phone number
- IP address
How we get the personal information and why we have it
The personal information we process is provided to us directly by you for the following reason:
- Completion of ACED Clinic Cambridge Enquiry Form (name, email address, enquiry details, phone number)
We also receive personal information indirectly, from the following sources in the following scenarios:
- Mailchimp form submission (IP address)
We use the information that you have given us in order to respond to your enquiry and add you to our mailing list if you request this.
We may share this information further if you consent to join the ACED Cohort. The details of this information sharing are covered by the process of consenting to join ACED Cohort and as such are beyond the scope of this policy.
Under the General Data Protection Regulation (GDPR), the lawful basis we rely on for processing this information is:
- Your Consent. You are able to remove your consent at any time. You can do this by contacting add-tr.acedcambridge@nhs.net.
How we store your personal information
If you do not subscribe to our newsletter, your information will be securely and temporarily stored on MailChimp, then transferred to secure NHS servers and restricted access research management systems and permanently deleted from MailChimp. If you do subscribe to our newsletter, your information will be stored on MailChimp until you unsubscribe or we cease to hold ACED Cohort records, whichever occurs sooner.
We will keep your personal information for 10 years after the end of the ACED Cohort study. We will then dispose your information by deleting all associated records.
Your data protection rights
Under data protection law, you have rights including:
Your right of access: You have the right to ask us for copies of your personal information.
Your right to rectification: You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure: You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing: You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to withdraw consent: You have the the right to withdraw consent for the processing of your personal information in certain circumstances.
Your right to data portability: You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

You can also complain to the ICO if you are unhappy with how we have used your data.
